Las úlceras por presión (UPP) constituyen un importante problema de salud pública por las grandes repercusiones negativas que tienen en el paciente y en el sistema de salud. En concreto la región sacrococcígea es considerada una de las zonas más vulnerables por soportar una gran presión y si además se le suman otros factores, como la inmovilidad, una desfavorable nutrición o incontinencia urinaria o fecal, se garantiza la instauración de una UPP. Se presenta el caso de una mujer de 85 años con antecedentes cardiovasculares, respiratorios y digestivos, con una UPP secundaria a una hospitalización por una fractura de cadera. La valoración enfermera se realizó mediante el modelo de Necesidades Humanas de Virginia Henderson. Para la etapa diagnóstica se utilizó la taxonomía NANDA y para el establecimiento de resultados e intervenciones, las taxonomías NOC y NIC, respectivamente. Los diagnósticos enfermeros identificados fueron “Deterioro de la integridad cutánea”, “Riesgo de cansancio del rol del cuidador” para su cuidadora principal y “Ansiedad” como problema de colaboración. Se consiguió incrementar la puntuación de todos los indicadores, alcanzar la epitelización del tejido de la UPP, disminuir la ansiedad de la paciente aumentando su satisfacción personal y favorecer el aprendizaje de estrategias de afrontamiento a su cuidadora principal. |

Pressure ulcers (PUs) represent a major public health problem, due to the great negative impact on patients and the health system. Specifically, the sacrococcygeal region is considered one of the most vulnerable areas because it supports great pressure; and if other factors are added, such as immobility, unfavorable nutrition or urinary or fecal incontinence, PU development will be guaranteed. We present the case of an 85-year-old woman with a history of cardiovascular, respiratory, and GI conditions, with a PU secondary to hospitalization for hip fracture. Nursing assessment was conducted with the Human Needs Model by Virginia Henderson. NANDA taxonomy was used for the diagnostic stage, and NOC and NIC taxonomies were used respectively for determining results and interventions. The nursing diagnoses identified were: “Impaired Skin Integrity”, “Risk of Caregiver Role Strain” for her main caregiver, and “Anxiety” as a collaborative problem. An increase in the scores for all indicators was achieved, as well as PU tissue epithelialization, a reduction in patient anxiety by increasing her personal satisfaction, and her main caregiver was encouraged to learn coping strategies. |
