En la muerte súbita cardiaca (MSC) por fibrilación ventricular idiopática (FVI) hoy sólo podemos hacer prevención secundaria y el tratamiento de elección es el desfibrilador automático implantable (DAI).<br/>A pesar del beneficio que para el paciente con riesgo de presentar arritmias malignas supone el implante de DAI, éste no está exento de provocar efectos adversos y complicaciones.<br/>En los pacientes con daño cerebral adquirido (DCA) tras episodio de MSC por FVI, el DAI debería aportar más esperanza de vida sin perjuicio en su calidad ya mermada por el proceso neurológico.<br/>Los profesionales médicos implicados en la atención a estos pacientes que, además del problema de las arritmias malignas tienen el grave problema del DCA, en ocasiones se encuentran ante decisiones terapéuticas difíciles de tomar, al estar implicadas cuestiones éticas, como en el caso que exponemos a continuación.<br/>Al ser el DCA problema de gran repercusión, tanto para la calidad de vida del paciente como la de su familia, Enfermería juega un papel importante en el cuidado y seguimiento de estos pacientes, así como en la educación sanitaria y apoyo a sus familias. |

With regards to sudden cardiac death (SCD) due to idiopathic ventricular fibrillation (IVF), nowadays it is only possible to perform secondary prevention being an implantable cardioverter-defibrillator (ICD) the preferred decision.<br/>Despite the benefit that an ICD brings for patients at risk of suffering malign arrhythmias, it is not exempt from involving adverse events or complications.<br/>In patients with Aquired Brain Injury (ABI), after an episode of SCD due to IVF, the ICD shoul expand life expectancy avoiding a negative impact on patient´s quality of life, already reduced provided the neurological process.<br/>Helthcare practitioners involved in the attention of these patients, that in addition to the malign arrythmias add ABI, usually face difficult therapeutic decisions due to ethical considerations being involved, like in the case reported in this document.<br/>Due to the great impact that the ICD has in the patient´s quality of life, as well as in his family´s, Nursing plays an<br/>important role in the care and follow-up of these patients, as well as in the support and health training of their families. |
