La creciente complejidad de los procedimientos terapéuticos, la especialización y segmentación de la asistencia sanitaria y la existencia de una cultura profesional sanitaria que tiende a ocultar en lugar de favorecer la comunicación abierta de los errores asistenciales y sus causas, son factores que predisponen al aumento de la incidencia de errores de medicación.<br/>Con el propósito de identificar y analizar los errores en la administración de fármacos en el ámbito de la Atención Primaria e implantar sistemas de notificación y vigilancia, se llevó a cabo un Análisis Modal de Fallos y Efectos (AMFE) sobre “Administración de fármacos”. Participaron profesionales de cinco centros, formado por tres enfermeras, un médico de familia, un pediatra y la directora de calidad del Grupo <br/>Sagessa. De entre todos los procesos relacionados con esa intervención, la “administración del fármaco” fue el que obtuvo un Índice de Puntuación de Riesgo más alto, calculado sobre la severidad y la probabilidad del mismo. El análisis de ese procedimiento dio como resultado la puesta en marcha de varias acciones protocolizadas que están comenzando a ser evaluadas. |

The growing complexity of therapeutic processes, the specialisation and segmentation of healthcare and the existence of a healthcare professional culture that tends to hide instead of favouring open communication regarding errors and their causes in the provision of care, are factors predisposing to an increase in the number of errors when administering medication.<br/>With the aim of identifying and analysing errors in the administration of drugs within the setting of primary care and also to implant notification and surveillance systems, a Modal Analysis of Errors and Effects (AMFE) on the “Administration of Drugs” was carried out. Five centres participated in the survey, including nurses, a family practitioner, a paediatrician and a quality director from the Sagessa Group. Amongst all the processes relating to the intervention, the “administration of drugs” obtained the highest risk score calculated on the basis of severity and the probability of error. Analysis of this procedure resulted in the creation of several protocols that are currently being assessed. |
