Introducción: tortícolis muscular congénito (TMC), es una patología que se presenta desde el nacimiento hasta los primeros meses de vida. Se caracteriza por la postura en la que deriva, es decir, inclinación de la cabeza homolateral y rotación contralateral, debido a un acortamiento unilateral o fibrosis del músculo esternocleidomastoideo (ECM). Su incidencia es del 0.3-2.0%. Se ve asociada a plagiocefalia, displasia de cadera y otras deformidades. Existen condiciones que hacen que aumente el riesgo de padecer TMC, como son: presentación de nalgas, parto instrumental y niños primerizos. La historia de un nacimiento con dificultad, se ha registrado en el 30-60% de los pacientes con TMC. Existen varias teorías sobre la etiología, pero todavía hay escasa evidencia de cuál es el origen. Por eso es determinante llevar a cabo una buena valoración, para descartar posibles patologías asociadas más graves. TMC se subdivide en 3grupos: Grupo 1: presenta un tumor en el esternocleidomastoideo (ECM). Consiste en un tumor palpable. Es una masa móvil y dura dentro del cuerpo del ECM que se nota al nacer. Es la presentación más común. Grupo 2: conocido como tortícolis muscular (TM), consiste en el tortícolis con acortamiento del ECM, pero sin tumor palpable. Grupo 3: es un tortícolis postural (TP), sin tumor ni tensión del ECM.<br/>Objetivo: esta revisión bibliográfica tiene como objetivo ver qué tratamientos fisioterápicos pueden llevarse a cabo para tratar TMC.<br/>Material y métodos: Se ha realizado la búsqueda en las bases de datos: PubMed, PEDro y ScienceDirect. Como resultado de la búsqueda se han obtenido seis estudios, presentes en la revisión bibliográfica.<br/>Resultados: Como tratamiento más usado están los estiramientos pasivos de rotación e inclinación de cuello, junto con un programa de ejercicios e instrucciones de manejo para padres o cuidadores.<br/>Conclusión: para que un tratamiento sea eficaz, es importante llevar a cabo un diagnóstico precoz y derivarlo a tratamiento fisioterápico. Se necesitan publicaciones que den mayor evidencia sobre el tratamiento para TMC. |

Background: Congenital muscular torticollis (CMT), pathology which is presented from the birth until the first months of life. It is characterized by the posture drift; that is to say, the head inclination to one side and the rotation to the opposite side. This is due to a unilateral shortening or fibrosis of the sternocleidomastoid muscle (SMC). The impact is in between 0.3- 2.0 %, and it is associated to plagiocephaly, dysplasia of the hip (DDH) and other deformities. There are several conditions which increase the risk in children to suffer CMT; breech delivery, instrumental delivery and first time delivery. The circumstances of a difficulty delivery have been registered in 30-60% of the patients with CMT. Although there are a variety of theories about the etiology, there is still limited evidence about the origin. Therefore, it is determinant to carry out a good evaluation to rule out more possible serious pathologies associated. CMT subdivides in three different groups: Group 1: presentation of a tumor in the sternocleidomastoid. This consists of a palpable tumor. It is a mobile and hard mass inside the body of the sternocleidomastoid muscle which is noticed in the delivery. This is the most common manifestation. Group 2: it is known as muscular torticollis (MT). This consists in a torticollis with tension in the sternocleidomastoid but without a palpable tumor. Group 3: it is a postural torticollis (PT), without tumor or tension of the sternocleidomastoid.<br/>Purpose: this review has as an aim to see which physiotherapist treatments can be carried out to treat CMT.<br/>Material and Methods: the search has been realized in the subsequently databases: PubMed, PEDro and ScienceDirect. As a result of the research there have been obtained six studies presented in the bibliographic review.<br/>Results: The treatment most commonly used consists in a passive stretching rotational and lateral flexion of the neck, along with a program of exercises and handling strategies instructions for parents and caregivers.<br/>Conclusion: to obtain an effective treatment it is important to carry out an early diagnosis and start a physical therapy as fast as possible. In fact, further studies are necessary to determinate a mayor evidence of CMT treatment. |
