La Tenosinovitis de De Quervain (TDQ) es una tenosinovitis estenosante que se produce en el primer compartimiento dorsal de la muñeca. Es causada por deterioro en el deslizamiento de las vainas de los tendones del abductor largo del pulgar (ALP) y extensor corto del pulgar (ECP). La etiología de esta lesión incluye desde un trauma agudo a la realización normalmente de forma muy continuada de un ejercicio al cual no se estaba acostumbrado. Puede ser también debido al resultado de un microtrauma acumulativo. Debido al mayor uso de las tecnologías y especialmente de los teléfonos móviles y videoconsolas, nos encontremos cada vez más con un aumento de la incidencia de esta patología o “tecnopatía” por su asociación las Nuevas Tecnologías en sujetos que utilicen estos dispositivos durante varias horas al día, ya que para manejar dichos dispositivos, la solicitación mecánica se produce especialmente en el dedo pulgar y en la muñeca. El propósito de este estudio ha sido determinar si el mayor uso de videoconsolas y Smartphone influye en la aparición de la TDQ.<br/>En el estudio participaron 319 sujetos de entre 14 y 17 años, a los que se le realizó una encuesta de uso de videoconsola y Smartphone para posteriormente diagnosticarles la TDQ mediante la realización del test de Finkelstein. Se tomó el valor de P<0.05 para considerar los datos como significativos.<br/>El test de Finkelstein fue positivo en la muñeca dominante en 67 sujetos (21%) y en 24 sujetos en la mano no dominante (7,5%). Se encontró correlación entre el uso del Smartphone y la videoconsola al día y a la semana y la positividad en el test de Finkelstein en la mano dominante (p=0.000), y entre el mayor uso de la videoconsola al día y la semana y la positividad del test en la mano no dominante (p<0.05).<br/>Como conclusión podemos afirmar que la prevalencia de la TDQ en adolescentes está relacionada con que el mayor uso de estos aparatos electrónicos. |

De Quervain's tenosynovitis (TDQ) is a stenosing tenosynovitis which occurs in the first dorsal compartment of the wrist. It is caused by impairde gliding of the Abductor pollicis longus (APL) and Extensor pollicis brevis (ECP) tendon sheaths. The etiology of this lesion ranges from acute trauma to the usually realization of an excercise which is not used to. It may also appear to the result of acumulative microtrauma. Because of the increased use of technology and especially mobile phones and consoles, the incidence of this disease in subjects who use these devices for several hours per day cause for the mechanical stress occurs especially in the thumb and wrist.<br/>The purpose of this study was to determine if the increased use of consoles and Smartphone influences the appearance of the TDQ. The study involved 319 subjects aged between 14 and 17 years old, who maked a questionnaire of use console and Smartphone, before to diagnose TDQ by performing Finkelstein test on them. P value <0.05 was considered significant.<br/>Finkelstein test was positive in the dominant wrist in 67 subjects (21%) and 24 subjects in the nondominant hand (7.5%). It was noted that as daily and weekly frequency of mobile phone and console usage increased progressively, more people showed positive Finkelstein test in the dominant hand (p = 0.000), and between the daily and weekly increased console usage , more people showed positive Finkelstein test in the non-dominant hand (p <0.05).<br/>In conclusion we can say that the prevalence of the TDQ in adolescence is related with the increase of use of these electronic devices. |
