Objetivo: localizar la evidencia científica sobre la efectividad de la terapia manual en el esguince cervical. Material y método: se realizó una revisión sistemática de ensayos clínicos para comprobar la eficacia de la terapia manual en el tratamiento del esguince cervical. Se buscaron ensayos clínicos, aleatorizados o no, publicados desde el año 2003 en las bases Medline, PEDro y Cochrane. Se incluyeron ensayos que probaran la eficacia de la Terapia Manual sola o incluida en Programas Multimodales de Fisioterapia. Se excluyeron trabajos no experimentales o que no tratasen la Terapia Manual. Resultados: se localizaron 28 ensayos clínicos, de los cuales 11 fueron seleccionados (el resto se excluyó por no cumplir los criterios de selección). De ellos: 4 comparaban el uso del collarín con otras técnicas, 3 estudiaron la efectividad de un tratamiento asesorado y supervisado de ejercicios, uno de ellos comparaba la educación por un médico general y la educación y consejo dado por los fisioterapeutas, otro estudiaba como incidían ejercicios sobre la ATM en la mejora de la sintomatología del latigazo cervical, uno estudiaba la eficacia de la manipulación dorsal y otro comparaba un protocolo de manipulaciones y un tratamiento de Fisioterapia convencional. Conclusiones: existen muchos tratamientos manuales para los sujetos expuestos a esta patología, pero pocas intervenciones evidenciaron efectos positivos. Dentro de estas limitaciones podemos decir que las manipulaciones están asociadas a una mejora de la sintomatología del esguince cervical y que además es recomendable una movilización activa precoz. |

Objective: to locate the scientific evidence on the effectiveness of manual therapy in cervical sprain. Material and method: A systematic review of clinical trials was carried out to verify the efficacy of manual therapy in the treatment of cervical sprain. Clinical trials, randomized or not, published since 2003 in Medline, PEDro and Cochrane databases were searched. Trials testing the efficacy of Manual Therapy alone or included in Multimodal Physiotherapy Programs were included. Non-experimental studies or studies that did not deal with Manual Therapy were excluded. Results: 28 clinical trials were located, of which 11 were selected (the rest were excluded because they did not meet the selection criteria). Of them: 4 compared the use of the neck brace with other techniques, 3 studied the effectiveness of an advised and supervised treatment of exercises, one of them compared education by a general practitioner and the education and advice given by physiotherapists, another studied how they affected exercises on the TMJ in the improvement of the symptoms of whiplash, one studied the efficacy of dorsal manipulation and another compared a manipulation protocol and a conventional Physiotherapy treatment. Conclusions: there are many manual treatments for subjects exposed to this pathology, but few interventions showed positive effects. Within these limitations, we can say that the manipulations are associated with an improvement in the symptoms of cervical sprain and that early active mobilization is also recommended |
