Recuerdo histórico: el uso de la episiotomía ha sido objeto de múltiples controversias a lo largo de la historia. Nuestro objetivo es analizar el estado actual del conocimiento respecto a la práctica de la episiotomía y al papel que la Fisioterapia obstétrica puede jugar en relación a ella. Evidencia científica actual: los estudios científicos actuales demuestran que la episiotomía rutinaria no constituye un factor protector para el suelo pélvico femenino, y además puede producirle toda una serie de efectos perjudiciales. Por ello, actualmente la OMS recomienda su intervención restrictiva en alrededor del 10-20 % incluso en mujeres nulíparas. Sin embargo, como consecuencia de los resultados hallados en algunos estudios, existen opiniones divergentes. Alternativas terapéuticas posibles: a tenor de los resultados que aparecen en los escasos estudios científicos realizados hasta la fecha y de nuestra experiencia clínica, creemos que la Fisioterapia (masaje perineal, ejercicios de suelo pélvico…) puede desempeñar un papel crucial para conseguir una disminución efectiva de las tasas de episiotomía y desgarros perineales espontáneos. Conclusiones finales: la práctica de la episiotomía rutinaria de tipo «profiláctico» debe evitarse, abogando por su uso selectivo. La combinación de la episiotomía medial con parto vaginal mediante fórceps parece constituir el factor de riesgo más importante para el suelo pélvico femenino. Para reducir las tasas de episiotomía y desgarros perineales espontáneos sería interesante implementar medidas preventivas fisioterapéuticas avaladas científicamente en la práctica diaria con la mujer embarazada. |

Historical reminder: the use of episiotomy has been the subject of multiple controversies throughout history. Our objective is to analyze the current state of knowledge regarding the practice of episiotomy and the role that obstetric physiotherapy can play in relation to it. Current scientific evidence: Current scientific studies show that routine episiotomy is not a protective factor for the female pelvic floor, and can also produce a whole series of detrimental effects. For this reason, the WHO currently recommends its restrictive intervention in around 10-20%, even in nulliparous women. However, as a consequence of the results found in some studies, there are divergent opinions. Possible therapeutic alternatives: based on the results that appear in the few scientific studies carried out to date and our clinical experience, we believe that Physiotherapy (perineal massage, pelvic floor exercises...) can play a crucial role in achieving an effective decrease of episiotomy rates and spontaneous perineal tears. Final conclusions: the practice of routine "prophylactic" episiotomy should be avoided, advocating its selective use. The combination of medial episiotomy with vaginal delivery by forceps seems to be the most important risk factor for female pelvic floor. To reduce the rates of episiotomy and spontaneous perineal tears, it would be interesting to implement scientifically supported physiotherapeutic preventive measures in daily practice with pregnant women. |
