Las instrucciones previas son un documento que tiene por objetivo garantizar el derecho de los pacientes a ejercer su voluntad en el final de la vida. A lo largo de la historia hemos comprobado que el documento de instrucciones previas no debe constituirse en un fin, sino en un medio. Es imprescindible englobar este documento en un marco de actuación más amplio e integral, la Planificación Anticipada de Decisiones para que podamos realmente contribuir a garantizar la autonomía del paciente en el final de la vida. Atención Primaria es el nivel donde este proceso debe ser realizado. La Enfermería Comunitaria es el colectivo<br/>profesional que debe ejercer el papel protagonista en la Planificación Anticipada de Decisiones. La Enfermería Comunitaria, por conocimientos, por metodología, por habilidades, por actitudes y por valores profesionales, es el colectivo que mayor valor añadido aportaría al proceso de planificación anticipada de decisiones. La Administración debe promover la implantación de este proceso favoreciendo la formación de los profesionales sanitarios, facilitando el procedimiento de otorgamiento e inscripción del documento a los ciudadanos; y mejorando la forma de acceder al contenido del documento desde la historia electrónica de salud para hacer efectivo ese derecho. |

Living Wills are a document that aims to ensure the right of patients to exercise their will in the end of life. Throughout history we have seen that the living will must not become an objective but a means. It is essential to include this document in a broader and<br/>comprehensive framework for action, the Advance Care Planning so that we can really help ensure patient autonomy at the end of life. Primary care is the level where this process must be done. Community Nursing is the profession who must exercise the leading role in the Advance Care Planning. Community Nursing, by knowledge, by methodology, by skills, attitudes and professional values is the collective that bring added value to the process of advance care planning. The Administration should promote the implementation of this process by promoting the training of health professionals, facilitating the process of granting and registration of the document to citizens; and improving way to access the contents of the document from electronic health history to enforce that right. |
