Summary, etc. La discinesia apical transitoria o Takotsubo es un síndrome descrito recientemente (año 1990). Predomina en mujeres posmenopáusicas y con antecedentes de estrés. Su sintomatología es muy similar a la del IAM: dolor precordial, cambios en el segmento ST del ECG y ligeras alteraciones enzimáticas. El cateterismo precoz muestra arterias coronarias normales y la imagen ecocardiográfica presenta discinesia apical transitoria en la fase aguda con normalización de la función ventricular días más tarde. El diagnóstico precoz del Takotsubo evita la administración innecesaria de agentes fibrinolíticos en pacientes con discinesia apical transitoria con elevación del segmento ST del ECG. Los cuidados de enfermería serán parecidos a los administrados a los pacientes con IAM e irán encaminados a aliviar el dolor, reducir la ansiedad y prevenir y alertar en caso de complicaciones. Presentamos un caso clínico junto con los Diagnósticos e Intervenciones de Enfermería que se pueden aplicar en este síndrome.

Summary, etc. The transient apical ballooning or Takotsubo is a recently described syndrome (in 1990). Occurs more often in postmenopausal women after a traumatic pychosocial or physical stressor. These signs and symptoms are suggestive of AMI: chest pain, ST- segment changes and the release of cardiac biomarkers. Coronary angiography reveals no significant coronary artery disease and the Echocardiographic image presents dyskinesis apical transient in the acute phase but left ventricular function improves days later. Prompt recognition of apical ballooning prevents the unnecessary administration of fibrinolytic agents to patients with apical ballooning who have ST-segment elevation. Goals of care are similar to those for patients with AMI: alleviating pain, reducing anxiety and preventing and alert of complications. We report a case with diagnosis and nursing interventions.
