• Introducción: Presentamos el caso clínico de un paciente de 74 años al que se implantó un marcapasos VDD y en dos años se rompió el aislante del electrodo siendo detectado y seguido por enfermería. • Objetivo: Documentar que la disminución de la impedancia del electrodo corresponde al deterioro o rotura del aislante, en nuestro caso por cizallamiento clavicular. • Caso clínico: El marcapasos se implantó en el paciente por bloqueo atrio-ventricular completo y síncopes. A los catorce meses del implante se detectó una bajada de 400 ohmios con umbral y RX de tórax normales; a los veintiséis meses la impedancia bajó de 300 ohmios y el paciente presentó estimulación pectoral intermitente, se decidió recambio completo de todo el sistema de estimulación. En la RX de comprobación post reimplante se evidenció la rotura del aislante en la zona donde se cruzan la clavícula y la primera costilla. El paciente estuvo informado en todo momento, dando lugar a que éste expresara sus temores y preocupaciones. Mediante escucha activa y con respuestas concretas, enfermería le ayudó a afrontar la situación ofreciéndole seguridad. • Discusión: La impedancia del electrodo baja cuando se rompe el aislante, en nuestro caso, por cizallamiento clavicular. La programación en monopolar es interesante. Las roturas de los electrodos son poco frecuentes y son mayoría en los implantes por subclavia. La atención de enfermería es muy importante para el enfermo y familia. |

• Introduction: We report the case of a 74- year-old patient with a VDD pacemaker implanted two years before: rupture of the electrode insulation was detected and followed by nursing staff. • Objective: To report that decreased electrode impedance corresponds to deterioration or rupture of the insulation, in our case due to clavicular shearing. • Case report: The pacemaker was implanted because of complete atrioventicular block and syncope. Fourteen months after implantation, a decrease of 400 ohms was detected, with normal threshold and chest X-ray; after twenty six months the impedance was lower than 300 ohms and the patient showed intermittent pectoral stimulation; it was decided to change the whole stimulation system. In the post re-implantation X-ray, the insulation rupture was observed in the area where the clavicle and the first rib intersect. The patient was kept informed at all times, which allowed him to express his fears and worries. Through listening closely and giving clear answers, nursing staff were able to help him face the situation while offering assurance. Discussion: Electrode impedance decreases when the insulation ruptures, in our case caused by clavicular shearing. The programming in monopolar devices is interesting. Electrodes ruptures are rare and mostly caused by subclavian implantation. The attention given by nursing staff is very important in helping both the patient and the family through times of stress. |
