• Introducción y objetivos: La hipotermia terapéutica es una práctica clínica recomendada en la cadena de supervivencia<br/>tras parada cardiaca recuperada, dado su efecto neuroprotector. Objetivo general: conocer el porcentaje de cumplimiento de<br/>los tiempos de actuación durante la hipotermia terapéutica, tras parada cardiaca recuperada. Objetivos específicos: describir<br/>el perfil del paciente y analizar si los factores de demora interfieren en los tiempos.<br/>• Material y métodos: Estudio retrospectivo observacional en pacientes que, tras parada cardiaca recuperada de origen<br/>cardiológico, fueron sometidos a hipotermia terapéutica, en un hospital de tercer nivel entre 2010-2013.<br/>• Resultados: Se incluyeron 43 pacientes: el 93% sufrió parada cardiaca extrahospitalaria; el 86% fibrilación ventricular como<br/>primer ritmo; y el 77,5% secundarias a síndrome coronario agudo con elevación ST. Tras recuperar circulación espontánea,<br/>iniciaron hipotermia en <6h 39 pacientes 90,7% (218 ± 135min). En 34 casos 79,1% se alcanzó temperatura diana en<br/><4h (180 ± 115min). Consiguieron temperatura objetivo en <6h, 19 pacientes 44,2% (398 ± 155min) tras recuperación de<br/>la circulación espontánea. Los pacientes con reanimación prolongada tuvieron menor porcentaje en el cumplimiento del<br/>intervalo inicio hipotermia - temperatura objetivo (2,9% vs. 44,5%; p<0,01). Los sometidos a angioplastia tuvieron mayor<br/>porcentaje en el cumplimento del intervalo de tiempo recuperación circulación espontánea - temperatura objetivo (82,4%<br/>vs 50,0%; p=0,05). La temperatura de ingreso fue significativamente menor en los pacientes que alcanzaron la temperatura<br/>objetivo en <6h (34,4ºC vs 35,3ºC; p<0,01) tras recuperar circulación espontánea.<br/>• Conclusiones: Los pacientes tratados con hipotermia terapéutica presentan un porcentaje elevado en el cumplimiento de<br/>los intervalos de tiempo estudiados según las guías clínicas. Ser sometido a angioplastia o tener una menor temperatura de<br/>ingreso se asocia a mayor cumplimiento de la temperatura objetivo. Por el contrario, la reanimación prolongada es motivo de<br/>incumplimiento |

• Introduction and objectives: Therapeutic hypothermia is a recommended clinical practice in the survival chain after recovery<br/>from cardiac arrest, given its neuroprotective effect. General objective: to know the percent compliance of the times of action<br/>during therapeutic hypothermia after recovery from cardiac arrest. Specific objectives: to describe the patient’s profile and<br/>analyze whether delay factors interfere with the times.<br/>• Materials and methods: Observational retrospective study in patients who underwent therapeutic hypothermia in a third<br/>level hospital between 2010 and 2013 after recovering from a cardiological cardiac arrest.<br/>• Results: 43 patients were included: 93% had an out-of-hospital cardiac arrest; 86% ventricular fibrillation as first rhythm;<br/>and 77.5% secondary to acute coronary syndrome with ST elevation. After return of spontaneous circulation, hypothermia<br/>was initiated in 39 patients (90.7%) in <6 h (218 ± 135 min). The target temperature was reached in <4 h (180 ± 115 min) in<br/>34 cases (79.1%). 19 patients (44.2%) reached the target temperature in <6 h (398 ± 155 min) after return of spontaneous<br/>circulation. Patients with prolonged resuscitation showed a lower percent compliance of the initiation of hypothermia – target<br/>temperature interval (2.9% vs. 44.5%; p<0.01). Those undergoing angioplasty showed a higher percent compliance of the<br/>return of spontaneous circulation – target temperature time interval (82.4% vs. 50.0%; p=0.05). The temperature at admission<br/>was significantly lower in patients who achieved the target temperature in <6 hours (34.4ºC vs. 35.3ºC; p<0.01) after return of<br/>spontaneous circulation.<br/>• Conclusions: Patients treated with therapeutic hypothermia show a high percent compliance of the time intervals studied<br/>according to the clinical guidelines. Undergoing angioplasty or having a lower temperature at admission is associated with a<br/>higher compliance of the target temperature. Conversely, prolonged resuscitation is a cause of non-compliance. |
