Summary, etc. La cirugía laparoscópica prolongada, sobre todo en posición de litototomía y mediante el uso de perneras, puede asociarse al síndrome compartimental en pierna sana. Presentamos el caso de un varón de 57 años que, tras una prostatectomía radical laparoscópica, desarrolló un síndrome compartimental bilateral en las piernas que necesitó de fasciotomía urgente de los cuatro compartimentos. El conocimiento de este síndrome asociado a la cirugía laparoscópica prolongada es esencial para sospechar esta complicación, realizar un diagnostico precoz y un tratamiento quirúrgico inmediato. Solo así se evitarán las graves secuelas de una demora en el diagnóstico

Summary, etc. Prolonged laparoscopic surgery, mainly in the lithotomy position and the use of leg cuffs can be related to well-leg compartment syndrome. We present the case of a 57 year-old male that developed a bilateral compartment syndrome after a radical laparoscopic prostatectomy that needed emergent four-compartment fasciotomy. The knowledge of this prolonged laparoscopic surgery related syndrome is essential for a high index of suspicion, early diagnosis and immediate surgical treatment. Being this the only way of avoiding the severe sequels of a delayed diagnosis.
