La prevención secundaria del cáncer de cérvix se realiza mediante programas de cribado. La posibilidad de la prevención primaria mediante vacunas es una realidad desde mediados de 2006. Existen dos vacunas, una tetravalente y otra bivalente, ambas han demostrado poder prevenir lesiones precursoras del cáncer cervical con pocos efectos secundarios. La edad vacunal prevista para todas las niñas, 12 años, implicará problemas asistenciales. A largo plazo cabe esperar una drástica reducción del cáncer cervical y modificaciones en el sistema de cribado actual. Se esperan resistencias entre los padres según su origen étnico, predisposición moral o religiosa, la falta de información, por miedo a fomentar la promiscuidad sexual sin proteger frente a otras infecciones de transmisión sexual, los posibles efectos secundarios y la temprana edad de vacunación. La aceptación es mayor cuando existen antecedentes familiares de cáncer, hijas mayores, apoyo de la pareja, obligatoriedad de la vacuna y si se retrasa la edad vacunal. Cabe esperar que las implicaciones socio-sanitarias de su aplicación serán importantes en España. La información facilitada por las matronas y enfermeras será fundamental para su correcta aceptación y aplicación. |

Secondary prevention of cervical cancer is done through screening programs. The possibility of primary prevention through vaccines has been a reality since mid-2006. There are two vaccines, one tetravalent and one bivalent, both have been shown to prevent cervical cancer precursor lesions with few side effects. The planned vaccination age for all girls, 12 years old, will imply assistance problems. In the long term, a drastic reduction in cervical cancer and changes in the current screening system can be expected. Resistance is expected between parents according to their ethnicity, moral or religious predisposition, lack of information, for fear of promoting unprotected sexual promiscuity against other sexually transmitted infections, possible side effects and the early age of vaccination. Acceptance is greater when there is a family history of cancer, older daughters, partner support, mandatory vaccine and if the vaccination age is delayed. It is expected that the socio-sanitary implications of its application will be important in Spain. The information provided by midwives and nurses will be essential for its correct acceptance and application. |
