Summary, etc. La rotura prematura de membranas (RPM) causa en España aproximadamente un 40% de los partos pretérmino y, como consecuencia, un 10% de mortalidad perinatal según la Sociedad Española de Ginecología y Obstetricia (SEGO)1. En este caso clínico se observó que una actitud expectante con los pertinentes controles ecográficos (índice de líquido amniótico [ILA] y peso estimado [PE]) y analíticos (recuento leucocitario y PCR) puede conseguir un periodo de latencia inusualmente largo, llegando a una edad gestacional más segura. La buena evolución posnatal de ambos gemelos justifica,al menos en esta ocasión,una actitud conservadora

Summary, etc. The premature rupture of membranes (PROM), according to SEGO (Spanish Association of Gynecology and Obstetrics), causes nearly a 40 % of preterm delivery and as a consequence a 10 % of neonatal mortality in Spain. In this case report it was observed that a conservative management with the appropriate ultrasound scan, the amniotic fluid index (AFI), andestimatedweight(EW)andanalytics,leucocytes’counts and C-reactive protein (CRP) controls can get an unusual long latency period, obtainingasafergestationalage.Theadequatedevelopmentafter the childbirth of both identical twins justifies, at least in this case, a conservative posture.<br/>
